About the Role  You’ll provide in-home OT services to older adults funded through Support at Home, completing: Functional assessments AT & home modification recommendations Falls prevention and safety planning Capacity-focused, strengths-based interventions Clear, clinically sound documentation aligned with current guidelines and reviews You’ll work independently in the community , with strong clinical backing, realistic caseloads, and systems that support quality decision-making — not rushed throughput.
